Limited appointment time, the ability of patients to do their own research which then needs to be discussed with practitioners, and the numbers of patients who are undiagnosed or misdiagnosed; these challenges and others make effective communications between patients and their practitioners more important than ever.</p>
<p>Good communications really boils down to two things: <strong>respect for each other, and the ability to manage expectations.</strong></p>
<p>The following will help you understand how to be a good communicator yourself, and what to expect from a practitioner who is a good communicator.</p>

<h3>A patient who is a good communicator:</h3>
<ul>
<li><strong>Will be mindful of the doctor&#8217;s limited time.</strong>While some references tell us a patient has an average of only 8-10 minutes per appointment with his doctor, other references say the average is 16-20 minutes. The discrepancy may be due to the kind of visit, whether the doctor is primary care or a specialist, or even health insurance coverage. Regardless of the difference, it makes most sense for us patients to prepare ahead for the probability that the visit will be shorter than we expect. <li><strong>Will be concise in his communication,</strong> preparing carefully for meetings with his practitioner. A well-organized patient prepares questions ahead of appointments, and sticks to the facts. With so little appointment time, you&#8217;ll want to be sure your doctor has all the important information about your problems, and has time to answer all your questions.</li>
<li><strong>Will ask the meaning of words and concepts he doesn&#8217;t understand. </strong>Doctors are trained to use a lexicon of med-speak that baffles us patients. General medical terms are used by all doctors or many specialties. Other words and concepts are specific to body systems, conditions, diseases or treatments. In all cases, you&#8217;ll walk away much more satisfied from your visit, having learned what you need to know, if you stop your doctor and ask for a definition or description when he uses a concept or term you don&#8217;t understand.</li>
<li><strong>If interrupted, will ask the doctor to stop and listen respectfully. </strong>Some studies say it takes only 23 seconds before a <a href="http://www.jaoa.org/cgi/content/full/105/1/13" target="_blank">doctor interrupts</a> his patient. <a href="http://jeromegroopman.com/" target="_blank">Dr. Jerome Groopman,</a> author of<em>How Doctors Think, </em>states that doctors interrupt their patients within 18 seconds of the start of their conversation. <a href="http://patients.about.com/od/changingdoctors/f/Why-Did-A-Nurse-Doctor-Or-Other-Provider-Insult-Me.htm">If your doctor interrupts you, it can feel like an insult</a>. Politely ask him to listen to your entire list of symptoms, or to let you ask your entire question. Sometimes a simple gesture such as gently holding up your hand will alert your doctor to stop and listen to you.</li>
<li><strong>Will ask his doctor what to expect next. </strong>No matter what point you are in your transition through the system: before, during or after diagnosis or treatment, asking your doctor what happens next will help you understand what is going on immediately, and what your outcomes might be. For example, if your doctor says he is sending you for a medical test, you might ask what he expects the results will be, or what the possible outcomes might be, and what they would mean. If he can manage your expectations, you will have more confidence about the process and its outcomes.</li>
<li><strong>Will know which questions to ask the doctor, and which to save for others. </strong>Your doctor is the person who should answer any of your medical questions. But other questions, such as directions to a testing center, or the time of your next appointment, or where you should park your car, can be asked of others on the doctor&#8217;s staff. That conserves your short appointment time for the important, medical aspects of your care.</li>
</ul>
<h3>A doctor or practitioner who is a good communicator:</h3>
<ul>
<li><strong>Has respect for her patient. </strong>Good doctors understand that a sick or injured patient is highly vulnerable. Being respectful goes a long way toward helping that patient explain symptoms, take responsibility for decision-making, and complying with instructions.</li>
<li><strong>Has the ability to share information in terms her patients can understand .</strong> It&#8217;s OK to use med-speak and complicated terms, but they should be accompanied by an explanation at the same time.</li>
<li><strong>Doesn&#8217;t interrupt or stereotype her patients. </strong>It&#8217;s easy for all of us to interrupt when we know time is short or we are in a hurry, but a practitioner who is a good communicator knows that if it can&#8217;t be done right to begin with, it will need to be done over. Listening carefully and respectfully will go a long way toward better outcomes for the patient.</li>
<li><strong>Has the ability to effectively manage patients&#8217; expectations. </strong>By helping her patient understand what the next steps will be, and what the possible outcomes and their ramifications might be, the doctor can go a long way toward helping that patient understand his problem.</li>
</ul>
<p><strong>What to Do If Your Communications with Your Doctor Don&#8217;t Work Well</strong></p>
<p>Sometimes, despite our best attempts, we just can&#8217;t establish that rapport with our doctors. the problem may be with our own approach, or it could be the doctor&#8217;s communications style. Here are some resources for helping you decide what to do if you and your doctor don&#8217;t communicate well.</p>

Some say yes, that the term covers everything these days from warmly hospitable to bare minimalist, depending on the tastes (and budgets) of the sponsors. When a leading faith-based healthcare system commits to defining “patient-centered,” though, it can take on new meaning.</p>
<p>The motto “patients first” was the driving force behind the design conceived for the <a href="http://www.kmcnetwork.org/schuster_heart_hospital/" target="_blank">Benjamin and Marian Schuster Heart Hospital at Kettering Medical Center</a>, which is one of seven hospitals comprising <a href="http://www.khnetwork.org/" target="_blank">Kettering Adventist Healthcare</a> in Dayton, Ohio. In keeping with a religious denomination known for its emphasis on personal health and wholesome lifestyles, network administrators pushed for design innovation for “the good of the patient.” And they accepted a highly conceptual approach to make it work.</p>
<p>As regular <em>HEALTHCARE DESIGN </em>readers know by now, interior designer Jain Malkin, CID,AAHID, EDAC, directs one of the most conceptual design firms in the business. Malkin and her San Diego-based team at <a href="http://www.jainmalkin.com/" target="_blank">Jain Malkin Inc.</a> have become known for incorporating, and even creating, strikingly original features that enliven the patient/family experience. The 130,000-square-foot Schuster Heart Hospital, a multi-service facility that opened in September 2010, is one of the latest examples of this.</p>
<p><span id="more-1364"></span></p>
<p>Challenged by the Kettering system to express traditional human and religious values in the new facility, Malkin borrowed from the widely recognized concept of the five elements of nature—earth, wood, fire, water, and metal—to express the Creation theme that underpins the Adventist perspective. The symmetry with the new five-story structure was perfect; each floor could be designed to invoke a particular element of nature, opening up areas of creativity and imagination within each paradigm.</p>
<p>“Overall, we wanted to create a sense of peace and calmness,” says Brenda Kuhn, RN, PhD, Schuster’s director of nursing. “The five elements worked well for that, and we complemented this with the layout of the facility, decentralizing nursing into five pods per floor to bring the nurses closer to patients and families (Figure 1).” The challenge for nurses working in the new setting was huge, says Sally Sterzer, RN, BSN, nurse manager for Schuster 3 East. “Our nurses came from traditional nursing units, and we had to think outside the box to create new processes that would work. Everyone got on board because we emphasized that patients come first—everything we did was structured around that,” she says.</p>

<blockquote><p>As more nurses, pharmacists and physical therapists claim this honorific, physicians are fighting back. For nurses, getting doctorates can help them land a top administrative job at a hospital, improve their standing at a university and get them more respect among colleagues and patients. But so far, the new degrees have not brought higher fees from insurers for seeing patients or greater authority from states to prescribe medicines….</p>
<p>Dr. Roland Goertz, the board chairman of the American Academy of Family Physicians, says that physicians are worried that losing control over “doctor,” a word that has defined their profession for centuries, will be followed by the loss of control over the profession itself. He said that patients could be confused about the roles of various health professionals who all call themselves doctors.</p>

<p>Una gran parte de los enfermos con Alzheimer u otras demencias acabará en residencias geriátricas. En los últimos tiempos se ha abierto camino una corriente de arquitectura que busca cubrir sus necesidades, distintas de las del resto de los ancianos. Sus diseños ayudan a los afectados a <strong>sentirse casi como en casa</strong>.</p>
<p><a href="http://thedoctorfactory.files.wordpress.com/2011/09/arq.jpg"><img class="alignleft size-medium wp-image-1358" title="arq" src="http://thedoctorfactory.files.wordpress.com/2011/09/arq.jpg?w=300&#038;h=200" alt="" width="300" height="200" /></a>Los centros para personas mayores construidos a la &#8216;antigua usanza&#8217; siguen existiendo, pero cada vez son más las organizaciones sociosanitarias y los arquitectos que apuestan por desarrollar edificios especializados. Algunos de estos proyectos se han presentado en el<strong>Congreso Internacional de Alzheimer</strong> celebrado en Madrid el 22 y el 23 de septiembre.</p>
<p>En palabras de Manuel del Río, de Del Río-Ferrero Arquitectos, se aprecia una &#8220;<strong>creciente sensibilización</strong>&#8221; hacia la arquitectura y el diseño sociosanitario. Su estudio ha elaborado el proyecto del centro Villafal, que se construirá en Paracuellos del Jarama (Madrid) a iniciativa de una asociación de familiares de enfermos, Afal Contigo.</p>
<p>En la nueva residencia madrileña, los afectados y sus familiares se encontrarán, en primer lugar, con un acceso al edificio alegre y luminoso. Las habitaciones, consideradas &#8220;el reducto más íntimo&#8221;, se personalizarán con objetos y fotografías de su vida anterior. &#8220;Cada elemento del mobiliario se diseña psicológica y ergonómicamente&#8221;, apunta del Río.</p>
<p>Madrid ya cuenta con una residencia de este tipo, situada en el PAU de Vallecas. Se trata del Centro Alzheimer Fundación Reina Sofía, diseñado por Estudio Lamela Arquitectos e inaugurado en 2007. Según el arquitecto Carlos Lamela, es &#8220;una mini ciudad&#8221; que muestra &#8220;una<strong>arquitectura tranquila, moderna y fácilmente construible</strong>&#8220;. En ella, los espacios exteriores cobran una gran importancia. Tanto, que ocupan casi la misma extensión que los interiores. Cuentan con huertos en los que los enfermos pueden trabajar.</p>
<p>Otra característica del proyecto de Lamela es que tiene en cuenta los<strong>problemas de orientación</strong> que experimentan los afectados por demencias. Cada &#8216;unidad de vida&#8217; (zonas con habitaciones) del edificio tiene un color diferente, para que sea más fácil de localizar.</p>
<p>Para Eckhard Feddersen, de Feddersen Architekten, lo fundamental es que &#8220;la gente se sienta segura y cómoda&#8221;. Con este objetivo en mente, las diversas residencias que ha proyectado &#8220;no parecen aeropuertos o escuelas&#8221;. Tampoco se asemejan a hospitales: &#8220;En mis casas no viven pacientes, viven personas&#8221;. Se decanta por edificios que tengan un aspecto de &#8220;<strong>apartamentos o pequeños hoteles mediterráneos</strong>&#8220;.</p>
<p><span id="more-1357"></span></p>
<p>Susan Black, arquitecta de uno de los estudios pioneros en centros para individuos con demencia, Perkins Eastman, coincide en la necesidad de desarrollar espacios que den sensación de hogar. Sin embargo, esto no siempre es posible. Las grandes residencias tienen la ventaja de que su mantenimiento es más barato porque comparten gastos fijos (de personal, servicios, mantenimiento, etc.).</p>
<p>Perkins Eastman planeó hace 20 años Woodside Place, una residencia situada en Pensilvania que ha servido como modelo para muchas otras. Se encuentra en medio del campo, está compuesta por <strong>pequeñas casas</strong>que son en gran medida autosuficientes, sus habitaciones tienen un estilo tradicional y se invita a los enfermos a que lleven sus propios muebles.</p>
<p><strong>La India cuenta con una luchadora</strong>, Radna Murthy, que consiguió abrir en abril de 2010 el primer centro para el cuidado integral de la demencia en dicho país. Situado en el centro de la ciudad de Bangalore, dispone de una clínica para la memoria, un centro de día, atención hospitalaria a corto y largo plazo y servicios de rehabilitación.</p>
<p>Un entorno familiar, íntimo, cómodo y que estimule a los pacientes. Son algunas de las características más apreciadas en estos centros. Una residencia puede albergar a 100 o incluso 1.000 afectados, pero se evita la masificación <strong>dividiendo el espacio en módulos</strong> y se realizan las distintas actividades (comidas, ejercicios de rehabilitación&#8230;) en grupos reducidos.</p>

<p><strong><img class="alignleft size-full wp-image-1355" title="salud" src="http://thedoctorfactory.files.wordpress.com/2011/09/salud.jpg?w=500" alt=""   />El presidente y cofundador de Microsoft, Bill Gates, y su esposa, Melinda, recibieron esta noche un premio de la prestigiosa Universidad Johns Hopkins por su &#8220;visión de liderazgo&#8221; para ayudar a salvar vidas en los países en vías de desarrollo. La universidad dijo en un comunicado que Melinda Gates recibió el premio en nombre de los dos filántropos durante una ceremonia en la Facultad Bloomberg de Salud Pública de la institución.</strong></p>
<p>&#8220;Ha sido un privilegio trabajar con Johns Hopkins en la última década. Bill y yo nos sentimos honrados de recibir este premio de un socio tan respetado por la Fundación&#8221;, dijo Melinda Gates en el comunicado, al referirse a la Fundación que lleva el nombre de la pareja.</p>
<p>&#8220;Johns Hopkins ha sido un líder detrás del progreso de la salud de las mujeres y niños. Gracias a su innovador enfoque y perseverancia, se han salvado millones de vidas en todo el mundo&#8221;, aseveró.</p>
<p>Durante la ceremonia, la universidad anunció el lanzamiento del Centro para la Salud Maternal y Neonatal, dirigido a los países en vías de desarrollo, donde persiste el problema de muerte prematura de parturientas y recién nacidos.</p>
<p>Cada año, 3,7 millones de niños mueren en su primer mes de vida, y unas 340.000 mujeres fallecen durante el parto. Casi todas las muertes ocurren en los países subdesarrollados, señaló el comunicado.</p>
<p>Con una fortuna de 59.000 millones de dólares, Bill Gates es considerado el hombre más rico de EEUU, según la revista Forbes, que además lo ha calificado como el &#8220;ser humano más magnánimo de la tierra&#8221;.</p>

<p>Bill Gates ha donado ya 28.000 millones de dólares para la lucha contra enfermedades como la polio o la malaria, es además el segundo hombre más rico del planeta, según esa misma publicación.</p>
<p>Es el primer año en que la Universidad Johns Hopkins otorga ese premio, aprovechando el 50 aniversario del Departamento de Salud Internacional de la institución, considerado el más grande del mundo.</p>
<p>En aras de promover mejoras en el área de salud pública en todo el mundo, el Departamento ha apoyado el desarrollo y diseminación de vacunas, programas de investigación de la diarrea y neumonía, de prevención del sida, así como programas de salud en los países pobres, entre otros proyectos.</p>

<p>It is estimated that by the end of the decade, close to $60 billion will be spent on healthcare construction, which implies that close to $300 million will be spent on art in healthcare. In the current economic climate, this investment cannot go unchallenged. Does the investment in art reap tangible, measurable, and accountable benefits to a healthcare organization?</p>
<p>Only if we are able to make this case can art dodge the risk of being value engineered. Inclusion of art is of particular significance in mental health settings, where patients are “perceptually” vulnerable. In this context then, art must be chosen carefully, creatively, and conscientiously.</p>
<p><span id="more-1349"></span></p>
<p><strong>Prior studies on art, mental health, and healthcare settings</strong></p>
<p>Today, there is a compelling body of evidence that makes a case for the impact of positive distractions, such as art, on clinical and behavioral outcomes, including reduced stress, anxiety, and pain perception. Many of these studies have been conducted in acute care settings in high-stress areas, such as ICUs, waiting rooms, and procedure rooms.</p>
<p>In the context of mental health, researchers have long speculated that the physical environment in which treatment occurs has an impact on both the treatment process and its outcome. The few empirical studies that evaluated the effects of psychoenvironmental design have shown a decrease in pathological behavior, selective behavior, and attitude changes among both staff and patients, with a significant decrease in the rate of patient violence, an increase in patient-staff interaction, and improved morale among both patients and staff.<sup>1</sup></p>
<p>However, a review of literature reveals there is a lack of studies in mental health settings that isolate the healing impact of particular environmental variables.<sup>2</sup><sup> </sup>We conducted a study that addresses this criticism by investigating the impact of a focused intervention using visual art.</p>

<p><strong>Our study</strong></p>
<p>The objective of this study was to determine the impact of different art conditions on levels of patient agitation in a psychiatric holding facility. Three distinct art conditions—abstract art, abstract representational art, and realistic nature—were introduced inside a psychiatric holding unit lounge (Figure 1).</p>
<p>The nature art displayed was a realistic photograph that followed the description of a savannah scene and previous guidelines for appropriate healthcare art.<sup>2</sup> The abstract-representational image was a painting similar to the photograph in content, color, and composition; but it was rendered in an abstracted style. Previous research has been criticized for the use of computer-generated imagery that controls for color and composition as lacking in artistic merit. Addressing this criticism, a comparable high art piece by Vincent Van Gogh was selected. The final art image was a chaotic abstract painting by Jackson Pollock with contrasting colors and non-representational content. This image was selected to provide a stark contrast to the other two images (Figure 2).</p>
<p>During the study, nurses on the floor were asked to observe patient behavior. At the end of the three-month period, data from the hospital on PRN medication prescribed and patient census was collected; nurses were not informed at the time of the study that PRN data would be analyzed. PRN is the anti-anxiety medication that is given to patients on a need-only basis for elevated levels of agitation.</p>
<p>Since this is a medication that is not routinely prescribed, it is a robust and objective measure of agitation. Researchers compared the ratio of PRN medication to patient census across three different art conditions and a control condition (with no art). PRN costs were provided by the hospital per PRN incident. Costs were compared across different intervention conditions to make a case for the impact to the financial bottom line of the hospital.</p>
<p>Finally, focus groups with 22 nurses were conducted to secure qualitative information pertaining to patient behavior and the staff’s perception.</p>

<p>El filósofo Emile M. Cioran explicó en numerosas ocasiones que su obra era fruto de un acto curativo, había comenzado a escribir para no volverse loco. El propio Dalí se definía como un paranoico a la vez que añadía: «Debo ser el único de mi especie que ha dominado y transformado en potencia creadora, gloria y júbilo una enfermedad mental tan grave».</p>
<p>El arte, en todas sus formas, no sólo es un modo de expresión, sino una herramienta terapéutica que en los últimos años ha experimentado un importante desarrollo como forma de complementar los tratamientos de diversas enfermedades y también como un medio de crecimiento personal.<br />
El empleo del arte como vehículo de comunicación, como exploración de uno mismo e, incluso, para exorcizar las miserias personales es tan antiguo como la propia Humanidad. Como Cioran y Dalí, otros muchos artistas se iniciaron en la música, la pintura o la danza como una vía de escape, como una cura.</p>
<p><span id="more-1335"></span></p>
<p>Estos personajes han practicado, de algún modo, lo que ahora se denomina arteterapia, a la vez que han creado obras sublimes. Sin pretender pasar a la historia y ni siquiera con la intención de crear algo bello, el resto de los mortales puede emplear estas formas de expresión para superar alteraciones emocionales, como terapia de apoyo en trastornos psiquiátricos e incluso para hacer más llevaderas enfermedades tan graves como el cáncer o el Alzheimer. De hecho, Dalí no podría considerarse «el único» porque esta técnica lleva más de 50 años ayudando a pacientes de Gran Bretaña y EEUU. Ahora llega también a España.</p>
<p>En el VII Congreso Europeo de Arteterapia, celebrado en Madrid en el mes de septiembre, Paola Luzzato, responsable del servicio de arteterapia del Memorial Sloan-Kettering Cancer Center (EEUU), describía el caso de una mujer de mediana edad diagnosticada de un cáncer de mama con metástasis. Esta paciente no aceptaba la idea de su enfermedad, rechazaba el tratamiento y hablaba continuamente de suicidio. Luzzato explicaba que una sola sesión sumergida en los entresijos de la expresión artística fue suficiente para que la enferma considerara, al menos, la posibilidad de continuar con la terapia antitumoral que su oncólogo le había indicado.</p>
<p>CREACIÓN. El vehículo de un cambio de actitud frente a la enfermedad como el que ha experimentado esta mujer puede ser la pintura, la escultura, la música, la danza, la escritura o el teatro (dramaterapia). Eso sí, no se trata de contemplar los trabajos artísticos de otros, sino de crear. Todos los especialistas señalan que lo importante es el proceso creativo y no que el resultado sea hermoso. También aseguran que muchas personas se resisten al principio porque piensan que no están dotadas para las artes, pero no es necesario tener ningún conocimiento previo ni aptitud particular.</p>
<p>Durante la gestación de las obras, los individuos entran en contacto con partes de sí mismos, con emociones que no se pueden explorar con la palabra. En el curso de este viaje toman conciencia de su situación y encuentran respuestas a su forma de enfrentarse a lo que les está ocurriendo.</p>
<p>Esta técnica, en sus diferentes modalidades, se emplea en EEUU y en el Reino Unido desde hace más de 50 años. Según Christine Lapoujade, Presidenta de la Asociación Europea de Arte Terapia (ECArte), el auténtico impulso se ha producido en los últimos cinco años: «Existen ya más de 30 universidades en toda Europa que proporcionan formación en esta disciplina y cada vez son más los países que se suman». Sin embargo, «sólo está reconocida como profesión en el Reino Unido», agregó a SALUD la presidenta de ECArte. En este país, el arteterapia, en todas sus formas, está integrado en el sistema general de salud como un servicio más a los pacientes y también en las escuelas para el apoyo emocional y psicológico de los niños.</p>
<p>Para definir el arteterapia habría que decir que es un auténtico crisol de disciplinas. En ella se unen, además de las bellas artes, la pedagogía, la sociología, la psiquiatría y el psicoanálisis. Carles Ramos, director del centro de formación en arteterapia Metáfora y coordinador del máster que se imparte en la Universidad de Barcelona sobre esta disciplina, la describe como «una forma de psicoterapia». Pero a diferencia del modo clásico, las sesiones no consisten en conversaciones sino que se podrían calificar más bien de juegos creativos de exploración.<br />
Sus posibles aplicaciones llegan a cualquier lugar donde un individuo necesite encontrar un modo de expresar o explorar sus emociones. Inicialmente, su uso se circunscribía a los trastornos psicológicos puesto que su gestación se produjo al abrigo de las teorías psicoanalíticas, pero con el tiempo se ha ido incorporando al manejo de otras patologías. «Tenemos casos de Parkinson muy avanzado en los que los pacientes han comenzado a dibujar. No son capaces, debido a los temblores, de hacer otras actividades, pero han logrado pintar», explica Ramos.</p>
<p>Existen múltiples trabajos a pequeña escala publicados en revistas especializadas, fundamentalmente anglosajonas, valorando los efectos del arteterapia. Sin embargo, el hecho de que no sea aún una profesión reconocida tiene como consecuencia que no existan grandes estudios concluyentes. De hecho, entre los objetivos de la responsable del Servicio de Medicina Integral del Memorial, uno de los centros líderes del mundo en el tratamiento del cáncer, está emplear las herramientas de la investigación científica para demostrar la aportación de todas las denominadas medicinas complementarias, entre ellas el arteterapia.</p>
<p>PSIQUIATRÍA. Aunque con menos años de bagaje, en España existen numerosos proyectos que buscan valorar sus efectos. Un ejemplo de ello es el trabajo llevado a cabo, en la Clínica Puerta de Hierro de Madrid, con enfermos mentales. «Es una experiencia piloto enmarcada en un proyecto de investigación a tres o cuatro años, fruto de un acuerdo entre la Universidad Complutense y el hospital», explica Raquel Fariñas, una de las dos arteterapeutas que, bajo la supervisión de una psiquiatra del centro, Belén Sanz, participaron en este programa. Entre sus múltiples observaciones está el hecho de que la evolución de la enfermedad queda claramente reflejada en las obras.</p>
<p>«La experiencia clínica es fantástica y los pacientes se sienten muy cómodos», afirma Sanz. El objetivo es que estos enfermos se mantengan en contacto con el entorno y devolverlos a la realidad, aseguran Fariñas y Sanz. Y el medio es la expresión y la exploración de sus emociones a través de la creación.</p>
<p>Sus resultados se presentarán en el VII Congreso Nacional de Psiquiatría que se celebrará en Palma de Mallorca. Aunque, según Sanz, el siguiente paso es redactar un protocolo más amplio que permita estudiar y medir la respuesta de los enfermos. En el centro de día de psiquiatría de este hospital, el arteterapia ha pasado a formar parte de las diferentes actividades que se llevan a cabo con los individuos que sufren trastornos mentales.</p>
<p>La mayoría de los programas de arteterapia que se ofrecen en la sanidad pública surgen de la colaboración entre las universidades donde se imparten másteres de posgrado en este área y los distintos centros. En Madrid acaba de arrancar en tres hospitales (Ramón y Cajal, La Paz y Clínico San Carlos) un acuerdo entre estas instituciones, la Universidad Complutense, la Fundación Coca-Cola y la asociación Mensajeros de la Paz para llevar los talleres de arteterapia a los niños que permanecen hospitalizados y a enfermos de Alzheimer.</p>
<p>Desde la Universidad de Barcelona se han promovido programas en la unidad de cuidados paliativos y de hematología del hospital Durans i Reynalds, en el servicio de desintoxicación del de Bellvitge y en toda la red de salud mental.</p>
<p>ALZHEIMER. El crecimiento del arteterapia responde a la tendencia que se observa en los últimos años de dar más relevancia a los aspectos emocionales o psicológicos de la enfermedad a la vez que se atiende el lado puramente biológico. Se ha aceptado ampliamente que el estado anímico, el estrés y la agresividad tienen un impacto importante en la salud física y en la evolución de los enfermos.</p>
<p>Esta técnica proporciona un canal de comunicación empleando la forma de lenguaje más ancestral e instintiva que son los símbolos. Las pinturas, los instrumentos musicales y el propio cuerpo —a través de la danza y del teatro— se transforman en frases que transmiten las vivencias más ocultas de la persona. «Es más fácil llegar a cuestiones a las que no podemos acceder por medio de la palabra», comenta Belén Sanz.</p>
<p>En muchos casos, los pacientes están incapacitados para expresarse verbalmente, como es el caso de algunas personas con enfermedades neurodegenerativas o de los niños autistas. «Evidentemente no tratamos de curar el Alzheimer. Nuestro objetivo es mejorar la calidad de vida y frenar el deterioro cuando es posible. Además, ayudamos al equipo sanitario y a la familia a manejar al paciente y les hacemos comprender que aunque no pueda recordar nada todavía tiene capacidad para sentir», explica Ramos.</p>
<p>De hecho, esta forma de afrontar o de detener la pérdida de facultades provocada por el Alzheimer encaja con las últimas investigaciones que demuestran que mantener cierta actividad mental retrasa el empeoramiento.</p>
<p>Aunque en arteterapia se puede trabajar en grupo o individualmente, la forma de tratar a cada paciente es específica. En este sentido, Patxi del Campo, musicoterapeuta y Director del Instituto Música, Arte y Proceso de Vitoria afirma que «trabajamos a partir de las necesidades [de cada persona]». En pacientes con Alzheimer que en las primeras fases de la enfermedad, conservan la memoria más lejana, pero tienen dificultades con los acontecimientos más recientes, «buscamos músicas que les evoquen el pasado y después empleamos esas canciones para incorporar cosas nuevas»,explica Del Campo.</p>
<p>En patologías como el Alzheimer y el cáncer «casi trabajamos más con las familias que con los propios pacientes», asegura Fabiola Hernán-Cortés, dramaterapeuta (o especialista en psicodrama) y psicooncóloga en el Hospital 12 de Octubre de Madrid. En cualquier caso, el objetivo es muy similar:ayudar a las personas del entorno a asumir la situación de su familiar y que, de este modo, puedan apoyarle de un modo más eficaz. Otros colectivos que se pueden beneficiar de esta práctica son las personas que han sufrido abusos, los ancianos y los niños con discapacidades, problemas de comunicación, de relación o que viven situaciones familiares conflictivas.</p>
<p>SANOS. Sin embargo, no sólo las personas que sufren alguna patología pueden disfrutar del uso terapéutico del arte. El ICE, un centro de formación de educadores de la Universidad Autónoma de Barcelona, financió talleres dirigidos por Sibylle Cseri, arteterapeuta plástica y Marta Moreno, dramaterapeuta, dentro de un programa de la Generalitat enfocado a trabajar la inteligencia emocional.</p>
<p>Los participantes eran los profesores de una escuela de primaria cercana a Barcelona. Según Cseri estos profesionales se sintieron muy satisfechos con la experiencia y encontraron en esta técnica un modo de liberarse del estrés y del síndrome del quemado, muy frecuente en este colectivo.</p>
<p>Viva Vigoreux es una pintora y arteterapeuta que atiende en su mayoría a profesionales liberales. «Estas personas, a pesar, de haber triunfado laboral y socialmente, sienten que han olvidado una parte de ellos mismos. En general se describen como prisioneros del entorno social y familiar», asegura Vigoreux. Las imágenes o las obras que crean les permiten reencontrarse con ellos mismos, con quiénes son realmente y alcanzar un cierto equilibrio en medio de jornadas frenéticas. «El arteterapeuta es simplemente un guía porque la búsqueda personal se hace al ritmo del individuo. Cada uno elige el camino y el grado de profundidad», aclara.</p>

<p><img class="alignleft size-medium wp-image-1331" title="img_wellbeing" src="http://thedoctorfactory.files.wordpress.com/2011/08/img_wellbeing.jpg?w=210&#038;h=270" alt="" width="210" height="270" />If you are immersed in the evidence-based design process, then you know that a connection to nature is at the core of how the design of the built environment impacts the reduction of stress/pain and replenishes the soul.</p>
<p>Landscape architecture is a profession that has created exterior places of wellbeing for centuries. Then why not use similar design principles from landscape architecture in the design of an interior? Have you tried? Where are the similarities in the core principles for designing a healing place outside versus inside? Is it truly about blurring the interior and exterior of a building’s experience or is there a secret formula we have yet to crack?</p>
<p>Let’s examine for a minute what would happen if we discarded all of our beliefs about how an interior should be designed and turned to what we know from a baseline of research about how to positively influence the human condition. What if from that baseline of knowledge we asked questions about how to create a safe, human-centered, efficient, effective, mobile, and restorative environment? Hypothesize, if you will, and explore a new set of design principles that inform a new design vocabulary.</p>
<p><span id="more-1330"></span></p>
<p>At the baseline, we would know that if the environment is to be safe/sustainable, we would follow design codes, guidelines, and the nuances in the current research that support the service at hand in a manner that would do no harm. If we wanted people to be drawn to our facility, we would analyze our market surveys and deliver a brand that delivered beyond its customers’ expectations. If we wanted the interior to be efficient, effective, and ready for full mobility, we would study electronic devices and the design interventions that improved their occupants’ output. If we wanted to reduce stress and pain, and replenish the human condition, we would add strong references to nature in our interiors.</p>
<p>So let’s say we did all that in this past building boom, do you think we will continue to develop hospitals in the same way, or will design teams be given the freedom to innovate a unique interior sensibility for healthcare? In pondering this question, I am certain that this new sensibility for healthy/healing interiors is about to emerge.</p>
<p>Hopefully, this economic downturn is allowing the thought leaders in the industry to wrestle with this same restlessness and, in the end, we will all benefit, not just in the healthcare sector but in all design sectors. My dream is for a new type of building enclosure that supports and enlightens the human condition. These new environments are “Places of Wellbeing” where we are safe; are mentally, physically, and spiritually supported; are efficient, effective, and mobile; and feel healthy and replenished.</p>
<p>In the last decade, healthcare projects that paid attention to the research included at least one healing garden, and some offered several gardens as a way to blur the line between inside and out. A few years ago, the introduction of daylight deep into a large institutional facility seemed costly and frivolous. Today, we have the business case that proves differently and have given design teams the freedom to use daylight as a standard light source for interiors. We brought landscape architecture into the building.</p>
<p>On the <a href="http://www.ohiohealth.com/dublinmethodist" target="_blank">Dublin Methodist Hospital</a> project in Dublin, Ohio, we worked closely with MSI on how at the window line (for which we had miles) to blur the edge of the interior and exterior landscaping. To do that, we placed trees, plantings, and water features in those interim spaces and created 13 gardens outdoors as options for waiting and reflection. Let’s push that concept further now and ask the questions imbedded in the concept of biophilia and see what our product manufacturers can learn from the cellular structure of living things in order to develop new materials for interior use.</p>
<p>Let’s look at the various design principles of landscape architecture to see how the design genre can be informed differently than in the past. Imagine a style not reflective of a former French king named Louis, but rather reflective of a form of vegetation. Would we use this newly found design vocabulary as a way to harness that powerful bond between man and nature, not man and wealth? And in the end, who would be the richer for the knowledge shared?</p>
<p>It is in this realm that I am about to spend the next effort of my professional career. It is absolutely evidence-based interior design; it is all about health and wellbeing; and it is going to launch from a baseline of certainty with an enlightened client base that isn’t afraid of innovation—clients like Cheryl Herbert of Dublin Methodist’s Pebble Project who allowed her design team to “run until apprehended” and innovate a new sensibility about the built environment.</p>
<p>I look forward to spending more time with colleagues in all disciplines, including landscape architecture, to explore greener, healthier pastures (or the interior correlation thereof) that we will come to refer to as Places of Wellbeing. <p>Aetna sends free mobile medical alerts and offers e-prescribing services to its Florida network doctors to better help coordinate health care. The insurance carrier has not yet said if it will expand the service nationwide, but it may do so if the Florida doctors embrace the new alert system.</p>

<p>Aetna said the services allow doctors to instantly access medical literature, as well as clinical and patient information from Aetna&#8217;s claims department, even while the doctors are with their patients. This will allow them to identify safety issues, talk with patients about missed tests or checkups and discuss health management programs.</p>

<p>The messaging service will also allow doctors to simplify and expedite prescription orders, so medications can be ready at the pharmacy immediately following their appointments.</p>
<p>&#8220;Access to patient and clinical information is critical for doctors to deliver high-quality, effective care,&#8221; said Bob Kropp, M.D., regional medical director for Aetna. &#8220;That&#8217;s why we&#8217;re putting integrated information into doctors&#8217; hands when it&#8217;s most effective &#8212; when the doctor is in the exam room with the patient.&#8221;</p>
<p>Aetna&#8217;s initiative is part of a larger movement to integrate mobile technology in clinics, hospitals and other health care settings. According to a study by Manhattan Research, 81 percent of all doctors will have adopted smartphones by 2012, and about half of them will use their devices to support everyday tasks in addition to coordinating patient care.</p>
<p>Already, more than <a href="http://www.mobiledia.com/news/95683.html">one-third of all doctors</a> surveyed nationwide are using smartphones to help with daily care, so Aetna network physicians are sure to welcome the new program.</p>
<p>The new Aetna programs should also help meet some of the growing demand for medical apps. Doctors aren&#8217;t only storing health records, but use the devices to help <a href="http://www.tabletedia.com/news/3546.html">educate their patients</a>. The Aetna program will help them do that even more, by giving them quick and easy access to medical information specific to each patient&#8217;s situation.</p>
<p>Many doctors also take advantage of the U.S. government&#8217;s <a href="http://www.mobiledia.com/news/95683.html">&#8220;Meaningful Use&#8221;</a>program, which allows doctors to claim Medicare and Medicaid expenses when they buy iPads and other tablets to control their paper records.</p>
<p>With the demand for apps growing, Aetna most likely won&#8217;t be the only insurance company to offer mobile services to network doctors. Insurance companies could also benefit by being able to access an instant record of patient care, which could help eliminate massive amounts of paperwork, and the efficiency of medical alerts could help to better coordinate and control costs of patient care.</p>
